A registered nurse or care coordinator from our staff will be visiting your home for an assessment of your health needs. Upon identifying what these needs are, we will discuss with you, your family and your attending physician about the necessary services to include in your individualized plan of care. Implementation of the care plan will be in the hands of our highly competent team of healthcare professionals.
